Transfer Location

If you transfer to another work location, this may result in a change to your medical option eligibility. If your transfer impacts your eligibility options, an enrollment change worksheet will be sent to your home. You will have 30 days to select another medical option.  Failure to do so could result in loss of medical coverage. If you change your coverage, this change will begin on your date of transfer.
